Lipid Therapy in Older Patients With Diabetes

Lipid Therapy in Older Patients With Diabetes

Summary:

A patient >75 years of age with type 2 diabetes is classified as having at least high cardiovascular risk—and often very high risk, depending on comorbidities and evidence of target-organ damage.

Guideline-Based Classification (ESC/EAS)

According to the European guidelines (European Society of Cardiology / European Atherosclerosis Society):

Very high cardiovascular risk, if at least one of the following applies:

  • Diabetes plus
    • established atherosclerotic cardiovascular disease (e.g. coronary artery disease, carotid plaque, peripheral arterial disease), or
    • target-organ damage (e.g. albuminuria, eGFR <60 mL/min, retinopathy), or
    • ≥3 additional major risk factors (e.g. hypertension, dyslipidaemia, smoking, obesity)
  • Diabetes duration ≥20 years

In clinical practice, many patients >75 years with type 2 diabetes meet at least one of these criteria.

 High cardiovascular risk, if:

  • Diabetes without target-organ damage
  • <3 additional risk factors
  • No known established atherosclerotic cardiovascular disease

Still clearly above a “moderate” risk category.

Role of Age >75 Years

  • Age is not a separate risk category, but it:
    • increases the absolute event rate
    • amplifies the clinical relevance of existing risk factors
  • In reality, a diabetic patient >75 years is almost never classified as only “moderate risk.”

Typical Consequences of Risk Classification

Lipids (LDL Cholesterol)

Risk category

LDL-C target

High risk

<70 mg/dL (1.8 mmol/L)

Very high risk

<55 mg/dL (1.4 mmol/L) + ≥50% reduction

Additional Considerations

  • Statin therapy: clearly indicated (intensity adapted to tolerability)
  • Blood pressure target: individualized, usually <140/80 mmHg
  • Glucose-lowering therapy: preference for agents with cardiovascular benefit (e.g. SGLT2 inhibitors, GLP-1 receptor agonists)
  • Benefit–risk assessment: particularly important in advanced age and frailty

Clinical Take-Home Message

A patient >75 years with type 2 diabetes is at least high risk—and in the presence of plaques or target-organ damage, almost always very high risk.
